To take the examination for Nationally Certified Psychiatric Technician, download and print out our application form:

Complete and mail this application form and your payment to the address at the top of the application form.

You must fill out the above form and either mail to: 1220 S St, Suite 100, Sacramento, CA 95811-7138, or fax to (916) 329-9145 your application for your order to be processed. Once you have completed your application form, make your payment using PayPal buttons below or mail a check to the above address.

Once a certification test is ordered, it cannot be transferred from one applicant to another.

Results can be expected no later than 30 days after completing the test and all other requirements. You should call to inquire on your certification status if it’s been over 30 days.

Certification Test Pricing

(Submit payment only with a completed application)

Basic -- Level 1 only	$139
Combined -- Level 1 plus Level 2, 3 or 4 at the same time	$159

Upgrade to Level 2, 3 or 4
-- At time of renewal (additional fee)	$30
-- At any other time	$45

Group Discount
For 10 or more applicants at one time, the discount rate is $119 for each Basic exam and $139 for each Combined exam.
